Important
These terms are written for UK consumers and reflect UK consumer protection law. Where any mandatory right applies, it takes priority over these terms. Nothing in these terms limits your statutory rights.
1. Agreement
This Services Agreement (“Agreement”) is made between Clean Slate Clinic Ltd (CRN:15100531) and the individual agreeing to these terms (“Client”). This Agreement sets out the terms under which Clean Slate Clinic will provide its services to the Client.
By agreeing to this document, the Client confirms acceptance of these Terms of Service, the Privacy Policy accessible here:UK Privacy Notice, and any required consents associated with the service. The Client acknowledges that this is a legally binding financial commitment.
2. Payment Options
The Client agrees to pay for the Clean Slate Clinic programme through one of the following options:
(a) One payment totalling £3,600
(b) 12 monthly payments of £350 totalling £4,200 (“Payment Plan”)
All prices are in GBP. Our clinical services are VAT exempt. If any item is VAT-able we will show VAT at checkout/invoice.
The first payment must be made before the first clinician appointment. For Payment Plan Clients, subsequent payments will be automatically charged each month to the nominated payment method. Failure to choose a payment option or to make the first payment before the first clinician appointment may result in delays or denial of services until the payment obligations are met.
The programme fee covers all three phases described in Section 3 (assessment & preparation; supervised withdrawal; recovery & maintenance), including clinician and prescriber appointments, care‑package materials, and access to specified group resources.
Any add‑on services you choose (see Section 11) are charged separately.
Any charges or costs charged by your chosen pharmacy for dispensing prescribed medication are payable by you and are not included in the programme fee.
3. Programme Description
The Clean Slate Clinic 12-month programme consists of three phases:
Phase 1: Assessment & Preparation
Two clinician assessments, one medical prescriber appointment, care package, bloodwork if required and withdrawal preparation over approximately two weeks.
Phase 2: Withdrawal
Medically supervised home-based withdrawal (detox) with daily clinical reviews and a further medical prescriber consultation, which includes discussion and prescribing of craving-reduction medication (if appropriate).
Phase 3: Recovery & Maintenance
Three weekly clinician reviews (one per week for 3 weeks), three monthly clinician reviews, three quarterly clinician reviews (9 appointments in total) and access to unlimited SMART Recovery meetings and online resources.
On completion of Phase 3 (12 months following programme commencement) the client will be discharged from the Clean Slate Clinic programme. Additional services can be purchased thereafter.
Clinical disclaimer: We cannot guarantee specific health outcomes. Care is delivered with reasonable skill and care by appropriately qualified personnel.
4. Cost Breakdown
This schedule is for reference only. Refer to Clause 7 for refund and cancellation terms.
Phase: Assessment & Preparation
Onboarding, assessments, and care package
Upfront Cost: £950
Payment Plan Cost: £1,050

Upfront Cost: £1,650
Payment Plan Cost: £1,850
Phase: Recovery & Maintenance
Weekly, monthly then quarterly reviews, SMART Recovery meetings and resource access
Upfront Cost: £1,000
Payment Plan Cost: £1,300
5. Payment Terms
Phase 2 and 3 are clinically integrated and form a single therapeutic programme. By proceeding to the first appointment in Phase 2, the Client agrees to undertake the full Programme, including Phase 3, and commits to the full programme fee, as outlined in Service Agreement.
Clients may choose to pay the programme fee in full upfront or via a payment plan. Choosing a payment plan does not convert the Programme into a month-to-month or cancellable service.
Once Phase 2 begins, you remain liable for the programme fee, subject always to your statutory rights and any reduction required where we save costs by not delivering parts of the programme. If you stop early, we will calculate what remains payable by reference to (i) services delivered and (ii) reserved capacity we cannot reallocate, taking account of any savings, and we will provide a breakdown on request.
Clinical pathway changes: If home-based withdrawal is not clinically appropriate or not required, we may adjust the programme (for example, by replacing withdrawal-phase appointments with additional recovery/support sessions). The overall programme structure and fee remain the same unless we agree otherwise in writing.
Missed or cancelled appointments (with less than 48 hours' notice) are forfeited and non-refundable. Additional sessions may be purchased separately, if required.
6. Payment Authorisation
The Client authorises Clean Slate Clinic to deduct payments from their nominated payment method. The Client must ensure the payment method remains valid with sufficient funds. If a payment method expires or is lost, it is the Client’s responsibility to provide an updated payment method prior to the next due payment.
7. Refunds and Cancellation Policy
To cancel, email services.uk@cleanslateclinic.com from the email address on your account, stating your name and that you wish to cancel.
The following refunds apply:
- Statutory cancellation (distance contracts): You may have a legal right to cancel within 14 days of entering into this Agreement. If you asked us to start providing services during that period and you cancel, you must pay a proportionate amount for services provided up to the time you notify us.
- Goodwill policy: If the Client cancels more than 48 hours before the first clinician appointment, they are eligible for a full refund.
- If the Client opts out after the first clinician appointment, and notifies Clean Slate Clinic in writing within two working days of the appointment, they will be charged £250.
- If you opt out during Phase 1 at any other time after the first clinician appointment, the Phase 1 charge of £950 applies.
General Refund Terms
Refunds are available only in the circumstances outlined above or in exceptional circumstances as determined by Clean Slate Clinic management.
If the Client proceeds to Phase 2, the Client remains liable for the programme fee in accordance with Clause 5 (subject to any rights and reductions required by consumer law).
Refunds will be processed within 14 days to the original payment method. Administrative fees will not exceed £50, and 'services rendered' will be defined as any appointments conducted, care packages delivered, or assessments completed.
8. Obligations and Warranties
The Client agrees to pay the full programme cost unless eligible for a refund under Clause 7. The Client acknowledges that this is not a subscription service but a legally binding financial obligation.
Clean Slate Clinic warrants that all personnel delivering services are appropriately qualified, trained, and experienced in accordance with UK healthcare regulations. Services will be provided with due care and in compliance with Clean Slate Clinic’s Privacy Policy and clinical guidelines.
9. Consequences of Non-Payment
(a) A late payment fee of £15 for payments over 7 days overdue, capped at a maximum cumulative amount of £90. Late fees/interest are intended to reflect reasonable costs of administering arrears and loss of use of funds.
(b) Interest on overdue payments at 3 percentage points per annum above the Bank of England base rate (simple interest).
(c) Suspension of services until overdue payments are resolved.
(d) Transfer to a collection agency for recovery with reasonable third-party costs actually incurred, borne by the Client.
10. Eligibility and conduct
You warrant that you are 18+ and are entering the Agreement for your personal use. If you are under guardianship, your legal representative must sign and provide evidence of authority. You agree to provide accurate information, attend scheduled reviews, and use prescribed medicines as directed.

To the extent permitted by law, our liability for loss or damage arising from the services is limited to the total amount you have paid for the programme/services giving rise to the claim. We are not responsible for delays or failures outside our reasonable control (e.g., internet outages, platform failures, courier delays). Your statutory rights are unaffected.
Nothing in this Agreement limits your rights under the Consumer Rights Act 2015, including the right to repeat performance or a price reduction where services are not provided with reasonable care and skill.

For urgent safety concerns, contact emergency services (111/999), and/or your GP.
15. Notices
We will send formal notices to the email you provided. Email is deemed received 24 hours after transmission unless a bounce‑back is received. Please keep your contact details up to date.
16. Changes to these terms
We may update these terms where necessary to reflect changes in law/regulation, clinical safety requirements, or operational changes that do not materially disadvantage you. We will notify you of any material changes. If a change materially disadvantages you, and you have not yet begun Phase 2, you may cancel before the change takes effect and receive any refund due under Section 7.
